Our staff will help you determine if you qualify for Medicaid, and case managers may be able to assist with the application process.

Billing and Reception staff are available to help you determine your costs. You may be eligible for a discount payment rate, based on your income.

Our Sliding Fee Scale for uninsured clients is available here.

If you think you will qualify, please complete this application on-line, or download the entire packet, print and return the application and proof of income in person or via fax to 907-375-3142.
